Southern Indian Health Council, Inc.’s medical team also includes a Community Health Nurse, Patient Advocate, and Community Health Representatives. These staff are also excited to be your partner in healthcare and actively be a part of your journey towards wellness. Our medical program includes this specialized program for our Consortium Tribal members to receive additional care and support. Services include:

Community Health Representative

The Community Health Representatives serve as a link between the clinical setting and the community to facilitate access to services and improve the quality and cultural competence of service delivery. They assist by increasing health knowledge of patients and communities through a broad range of activities such as transportation to health visits, outreach, community education, informal counseling, social support and advocacy. The Community Health Team is committed to improving and eliminating any barriers to care for our patients.

Patient Advocate

Our Patient Advocate guides and offers patient assistance to community members in navigating patients’ health services provided by SIHC and referrals made by SIHC. They establish and help maintain communication between patients, their families, clinicians and the health care system while offering additional assistance when needed, with the goal of improving health care outcomes and reduce health disparities for SIHC patients.
